Planning NDIS supports before a hospital discharge
A checklist for discharge planners and support coordinators to get supports ready on day one.
The first days at home after hospital are when supports matter most. A good discharge brings the hospital team, the participant, their family, the support coordinator and the provider together before the discharge date.
Before discharge
Contact the provider as early as possible, ideally as soon as a likely discharge date is known.
Share the discharge summary, current medication list and any new health supports, such as wound care, catheter care or PEG feeding.
Confirm the plan has funding for the supports needed, or start a plan variation early if needs have changed.
Check that equipment and home modifications are in place, or arranged, before the person arrives home.
Arrange any hospital-led training for workers, for example on a new feeding regime.
On the day
Make sure a worker is booked for the first visit and knows the arrival time.
Confirm medications are available at home.
Share a contact list: GP, hospital ward, community nursing, coordinator and family.
The first two weeks
Schedule a review of the support plan within the first week.
Watch for signs that more support is needed, and adjust quickly.
Keep the GP and allied health team in the loop.
